Job Description:

We are seeking a Service Coordinator Officer to join the Home Care Package team.

  • Develop and maintain co-operative working relationships with Consumers, carers/family, service providers, and Aged Care Advisors to enable efficient, reliable service coordination with a Customer First focus.
  • Manage daily enquiries and service changes for Consumers to meet their needs, being cognisant of the financial status of Consumer budget, and advise Aged Care Advisors of any significant service or financial care implications.
  • Identify situations of risk for Consumers or staff and promptly notify management.
  • Maintain electronic consumer records with a high degree of accuracy of all interactions relevant to service provision.

Job Requirements:

  • Ability to build cooperative working relationships with other service providers to promote positive working relationships.
  • Advanced ability to sensitively communicate with people from all walks of life and communication abilities whilst having understanding and empathy for their complex care needs
  • Experience working in a community service environment.
  • Well-developed problem solving and negotiation skills to provide advice to Consumers and their Carers, and to know when to refer to the Case manager.
  • Experience using a Consumer Information System such as CareLink
