Job Description:

In this role you will be working in the Community Care team to deliver high quality Community Care Services to ensure the assessed needs of frail aged older people, people with dementia, people with disabilities and their carers are met in a way that preserves or enhances their wellbeing.  You will also provide leadership, guidance and support to the Community Care Workers providing services to the target group within our community.

Job Responsibilities:

  • Through the Community Care Team, be responsible for the efficient and effective implementation of Council’s services and policies.
  • Ensuring appropriate provision, coordination, development and evaluation of Community Care Services in a defined geographical area
  • Providing day to day supervision and support to Community Care Workers and deploy staff flexibly, on a daily basis, to maintain the service provision roster up to date with clients’ requests.
  • Promoting teamwork and fostering exchange of ideas and participation of staff members to develop improvements in Community Care practices.
  • Assisting with the preparation of information for the annual budget estimates and monitoring of the budget for a geographically defined area.
  • Ensuring effective and efficient resource utilisation.

Job Requirements:

  • Post-secondary training/qualifications in a human services field.
  • Knowledge and awareness of issues relating to frail aged older people, people with dementia, people with disabilities and their carers.
  • Knowledge base in Community Care Services, Government policies, CHSP guidelines and funding requirements.
  • Skills in service administration, including service rostering, staff recruitment, supervision, staff development and deployment.
  • Leadership and management skills and an ability to develop a team and motivate staff.
  • Understanding and ability to implement personnel practices, including those related to Occupational Health & Safety and Equal Employment Opportunity legislation and policies and employees training and development.
  • Ability to communicate sensitively and effectively with older people, people with dementia, people with disabilities and their careers.
  • A current First Aid Certificate.
