Job Description:

We are seeking a Home Care Package Intake officer (Aged Care Advisor) to grow and maintain our Home Care packages sign ups who will be responsible for delivering and promoting AccessCare’s Home Care Packages on Councils behalf. Triaging and responding to consumer enquires and facilitating the signing on of new consumers. You will be a key driver of the organisations cultural change in placing customers at the heart of everything we do. You will be innovative, responsive to client feedback and able to quickly change work practices to address the significant changes in the aged care sector.

You will collaborate with referral partners, our brokered service providers and the broader staff,understanding processes and systems and being curious to new ways of doing things.

You will be joining the team at an exciting time of change, innovation and growth and working closely with an experienced, dedicated team.

Job Requirements:

  • High levels of organisation, ability to meet KPIs, promote positive problem solving when anomalies present
  • Understand the application of the Aged Care Quality Standards in a community setting
  • Well-developed written communication skills with a high degree of accuracy and efficiency.
  • Advanced ability to sensitively and at times assertively, communicate with people from all walks of life whilst having understanding and empathy for their complex care needs
