Job Responsibilities:

  • You will build and maintain relationships with a broad network of service providers to support older people, people who need assistance to live independently, their families and your colleagues.
  • You will encourage clients to make choices about their services and support their choices using a wellness and reablement approach to service delivery.
  • You will support a smooth intake of clients and seamless discharge and transition of clients who require services from other providers

Job Requirements:

  • Diploma of community development, case management, social work, disability services, aged care services or similar
  • Experience in client management, service navigation or client assessment in the aged care or disability field.
  • A strong knowledge and understanding of the service system and issues facing frail older people and younger people who need assistance to live independently.
  • Kindness, care and great communication skills
  • Sound, practical understanding of client-focussed service deliver
